Sadly qmail is very much outdated and not ready for the current world full of spam and virusses. qmail is one of the major causes of fake bounces: spammers or virusses using a fake sender address cause qmail to bounce mail to that fake address, causing an innocent bystander to receive enourmous amounts of mail. This is caused by qmail accepting any and all mail to all its local domain names, before checking weather it would accept it. Patches to correct this behaviour do exist and should be applied before trying to run a serious qmail server. Of course, you could also consider running a more modern MTA.
